E&I contract documentation available on this site is being provided for use by E&I members only and shall not be distributed and/or shared outside a member’s institution. Sharing of contract documentation outside the membership of the Cooperative is detrimental to its members and the Cooperative, as it impacts E&I’s ability to negotiate and maintain competitive agreements.
Complete and submit the Pathify Statement of Intent form. A Representative from Pathify will respond within 2-3 business days to discuss your specific needs, scope of work and services required. Contact your E&I Representative for help at any time.
Supplier
Once the SOI form is submitted, Pathify will consult with the members to develop an Order Form. Multiple meetings may occur depending on the services requested. Member and Supplier will sign the Order form when ready to move forward and initiate services. E&I will also follow up to ensure successful enrollment.
